How can I apply Duplicate passport in case Lost or Theft
In case, if you have lost your passport or your passport is stolen, then you need to apply for a duplicate passport. Lost or theft of passport should immediately, be reported to the local police station and to the Passport Office (PO) or Indian Mission, if abroad. At the same time, you need to apply for a duplicate passport to your nearest Passport Seva Kendra or authorised passport application centres with an application form, with the following documents attached.
Proof of present address, proof of date of birth, affidavit explaining how and where the passport got lost/damaged, FIR in original, self-attested photocopy of first two and last two pages, including ECR/Non-ECR page of the old passport, if available, three passport size photographs, a detailed letter explaining the reason, if applying for a tatkal passport. You need to carry the original and one photocopy of all the documents mentioned.
It takes about 6 to 8 weeks to get a duplicate passport, however if someone needs to travel on a medical emergency or due to sudden demise in the family, then a limited validity passport can be issued requesting the passport officer. Under the tatkal provision, one can get a duplicate passport within 7 to 8 working days. While applying for a limited validity passport, it is mandate to furnish photocopies of the original passport. However, if someone does not have a valid photocopy of the original passport than one should have details like passport number, date of issue, date of expiry and place of issue while filling up the re-issue application form. You have to contact respective mission/post of that country if failed to provide this information. If your passport was about to expire or if you want to make any changes in your address or any other vital information, you have the liberty to request to make the necessary changes.
If the passport is lost or stolen while travelling, you need to contact respective mission/post of that country to issue an emergency certificate. This certificate is a one-way travel document valid for a return trip back to your country and requires an approval of the High commission. Once you reach your home country, an application for re-issuance can be applied.
The pocket pinch for issuing a duplicate passport of 36 pages is Rs. 2500/- and for duplicate passport of 60 pages is Rs. 3000/-. One has to incur additional of 2500/- if applying under tatkal scheme. Forms can be filled online or you can visit your nearest PSK. Payments cannot be made online however you have the facility to make a payment at the Passport centre itself when you come to submit your documents. Passport applicant should be present at the time of submitting the documents.
A very important point to note is, that it sometimes, becomes very strenuous to get a duplicate passport for the second time. PSK might reject the application of habitual losers of passport.
